Manuscript — Editorial Additions

Action:  Insert the probability/prediction paragraph into Chapter 3 after the seven layers.
Description:  The system predicts the next token using probability distributions. Not comprehension — prediction. A therapist operating from pattern recognition does the same thing. A present therapist holds the space open. The system cannot do that. People do not understand the difference. That is the gap. Draft completed. Awaits insertion and Chapter 3 reissue.

EC-02 — Skin graft line — Chapter 10  [COMPLETE]
Action:  Paul writes one sentence. Claude places it in Chapter 10.
Description:  Paul learned to speak again after cancer. A piece of his wrist is now his tongue. He learned the dialect of his reconstructed mouth before he learned the dialect of AI. CLOSED 25 May 2026 by V33/V34 adoption. Placed Ch 10 opening as part of substantial chapter rebuild. Chapter now opens on the 2017 cancer diagnosis, the consultant (“prepared to be unintelligible on the phone”), Paul’s reply (“don’t worry, I don’t use the phone”), the 50/50 odds, the surgery in full detail (8 hours, 3 surgeons, 6 days ICU, iPad communication, the “Please take me into [name held]” note, “A, B, C, D eventually came out”), the wrist graft, and the daily-reminder framing carrying process + good fortune + “it might come back” for cancer survivors. Description:  This was the placeholder for the listening pass that has now happened. Paul completed the listening pass on 20 May 2026, producing 7,168 words of structured editorial feedback (35 numbered items). EC-10 — Chapter 10 — Medium as Dialect flag  [COMPLETE]

Description:  The gap between typed and spoken conversation informs the dialect chapter. Paul decides on editorial pass whether it needs a new section. CLOSED 25 May 2026 by V33/V34 adoption. DECISION: Dialect (not Medium) confirmed by Ch 10 rebuild. Dialect retained throughout the chapter as the central concept — medium would have implied channel (text, voice, image) where the chapter actually teaches register, conventions, and rhythm. Description:  Closed by PC v6 Item 16 decision. Mind the Gap ends with Chapter 6. Chapter 6a does NOT get placed as a separate chapter. Instead, the directional defence material from Defending the Gap and the cascade material from 6a(4) integrate into a rewritten Chapter 6, with citation to Where Does The Shame Go? EC-13 — Working-document integrity audit (NEW 23 May 2026)  [OBSERVATION — ONGOING]
Action:  Audit working documents for hallucinations carried in compaction summaries. The OOAI catch.
Description:  On 23 May 2026, Paul caught an OOAI definition hallucination that had been silently sitting in the compaction summary at the start of the working thread. The hallucination defined OOAI as “Out Of AI — the discipline of recognising when the AI conversation cannot be the source of verification.” That definition is not Paul’s. OOAI = Outcome-Oriented AI, Paul’s coined term, equivalent to OpenAI /goal. The manuscript itself was already correct (lines 1759, 1777). The hallucination existed only in the compaction summary and would have propagated forward had Paul not caught it. This is exactly the failure mode the Handing Over a Monster Thread Methodology Paper warns about: the compacted summary is not the same as the live thread, and what hands over to the new collaborator is the artefacts, not the compaction. Working method lessons: (1) Treat compaction summaries as suspect, not authoritative. (2) Trust the artefacts on disk over the compacted memory. (3) Paul’s discipline of reading Claude’s outputs carefully and noticing when definitions drift is the failsafe. (4) The Calibration Brief’s rule “never guess silently” applies to compaction-loaded context as much as to in-session output.
